Before you apply
- This application form is only to request changes to individuals or organisation account holders. If you want to update a Crown land manager or statutory land manager details, submit your changes via the Reserve Manager Portal.
- Prepare the documents you will need to complete the online application form.
Supporting documentation
- If you are changing organisation details, upload a document as evidence that you are authorised to make these changes.
- Any change to the company name and ACN will need to be supported by an ASIC search.
- A document as evidence that you are authorised to act on behalf of the account holder, if applicable.

- The fields marked with an asterisk * are mandatory and must be completed in order to submit the form.
- After you have submitted your form, a copy of the application can be emailed to the address entered in the form or a copy can be saved or printed.
